Why do doctors use the term “grafts” instead of “hairs”?
If you’ve just started researching FUE hair transplants, you’ve probably come across the term grafts (or follicular unit grafts) instead of “hairs.”
For example, in this case, the patient received 2735 follicular unit grafts.
So why do surgeons use the term grafts instead of simply counting hairs?
Most people don’t realize that a “hair” is usually not just a single strand. What we refer to as a follicular unit (or graft) typically contains anywhere from 1 to 4 hair shafts. In other words, one follicle can produce 1, 2, 3, or even 4 hairs.
During an FUE procedure, this entire follicular unit is extracted and transplanted as a single entity. That’s why surgeons refer to grafts or follicles—because that’s the actual unit being moved, not individual hairs.
So how many hairs are actually transplanted?
This varies from patient to patient and depends largely on genetics. The common way to describe this is “hairs per graft.”
Here’s a simple example. Let’s say a surgery produced the following distribution of grafts
- 139 single-hair follicular units
- 535 two-hair follicular units
- 332 three-hair follicular units
- 20 four-hair follicular units
Total grafts (follicles):
139 + 535 + 332 + 20 = 1,026 grafts
Total hairs transplanted:
(139 × 1) + (535 × 2) + (332 × 3) + (20 × 4)
= 139 + 1,070 + 996 + 80
= 2,285 hairs
Hairs per graft:
2,285 ÷ 1,026 ≈ 2.2 hairs per graft
Why this matters
In general, the higher the hairs per graft, the greater the visual density—all else being equal.
However, a good hair transplant is not about chasing the highest numbers.
For example, when designing a natural hairline, it’s often better to use single- and double-hair grafts. This allows for a softer, more refined appearance and avoids a “pluggy” look. It also preserves multi-hair grafts for areas where density is more important—or for potential future procedures.
